Appointment Checklist
We encourage you to make your appointments as far in advance as possible. We recognize the importance of your time and will make every effort to meet your scheduled appointment. We ask your understanding when medical emergencies may occasionally cause us to delay or reschedule your appointment. Please notify us as soon as possible if you must cancel your appointment, or be unavoidably late. If you must cancel your appointment due to last-minute unforeseen circumstances, please let us know immediately. Effective January 1, 2009, Gessler Clinic, P.A. charges existing patients $30.00, and new patients $50.00, for any appointments not canceled at least 24 hours in advance.
Be sure to bring:
Your medical records
or make sure that we have received them
A list of all current medications
including herbal and over-the-counter medicines, and nutritional supplements
Photo I.D.
Your health insurance card(s)
You will also be asked to present your insurance card(s) on all subsequent appointments. If co-payments are required by your insurance, they must be made prior to seeing the doctor.
If patient is a minor child
and not accompanied by custodial parent, then a notarized statement for authorization of treatment must be brought in by the person with the child.
